Affiliates – in order to process any payments that are due to you as a result of an Affiliate Partner Commission you will need to do the following:
- Download a copy of W-9 (US) or FW8BEN (Non-US) – you can download a blank copy below
- Complete the form and sign
- Save as a JPG or SCAN a copy
- Email your ‘scan’ or ‘jpg’ to: don AT greatresults DOT com
We will then have your document on file with our accounting firm and mark your file as complete and ready for payment processing.
